Services include: a toll-free Breast Cancer Hotline for New Yorkers with and/or concerned about breast cancer and is staffed by professionally trained and supervised volunteers, all of whom are breast cancer survivors, and is available daily from 9am to 9pm. We provide licensed Social Work Services (in English and Spanish) which includes counseling, crisis intervention and referrals, in person, on the phone and via zoom. The program offers Support Groups which are facilitated for individuals and include groups for newly diagnosed, women with metastatic or Stage 4 disease, women under 40, Creative Arts, recurrence of breast cancer; we also provide two Spanish-language support groups: Cafecito which is a Survivorship group and Para Nuestro Bienestar for newly diagnosed women. Sisters United in Health (SUIH) is a coalition of local organizations that provides community outreach, health education, workshops and referrals for free or low-cost mammography and other health screenings to underserved communities. Serves as the liaison between the county government and its Asian population. Coordinates resources for the Asian community with the goal of educating, empowering and developing integration initiatives that will foster greater participation in Nassau County’s social, economic, and civic life. Our staff works with the County Executive’s Office and other County agencies on the research, analysis, and implementation of policy to ensure that the Asian community is afforded equal opportunities, meaningful engagement, and access to Nassau County government services. The agency provides language access assistance to constituents seeking information about existing community and/or services related to employment, health, housing, and educational concerns; Organize cultural events to promote the diversity of Nassau County and allow residents to feel included in the fabric of the county; helps promote full participation of Asian-owned businesses in competing for bidding contracts in Nassau County. Provides diagnostic evaluations and psychological services to adults, adolescents, children and families of diverse cultural backgrounds who are having, behavioral, emotional, learning or adjustment problems. Emphasis is placed on clarifying problem areas, developing realistic goals and providing active short-term treatment. The full psychological evaluation consists of a biographical data review, psychological testing and assessment, an interpretive conference and a written report. Psychotherapeutic programs for behavior change include: Anger management, stress reduction, parent skills training, marriage and family counseling, and counseling for adults and children who have been abused. Consultation with school districts and other professional organizations are also available to develop programs within school and professional settings. A 'Health Home' is a network of support services for Medicaid recipients in the community. Health Homes provide Care Management services to ensure all those involved in an individual's care are working together and sharing the information necessary to support a person's recovery. This means coordinating not just medical, mental health, and substance abuse services, but the social service needs of the individual as well. A Health Home can help individuals navigate the complex healthcare system, link to resources for finding affordable housing, food, clothing, childcare, and transportation, and advocate on behalf of the individual to ensure they're receiving the care they deserve. Good afternoon, Broome provides companionship through conversations and meets the social needs that older adults may benefit from as part of healthy aging.

Helps families dealing with homicide through individual and group counseling, assistance in dealing with the media, court process information and advocacy, help in accessing NYS Crime Victims Compensation, including burial benefits.

Handles civil cases which involve personal injury, property damage or liability, medical or dental malpractice, real estate actions, equity suits such as injunction proceedings, specific performance contract disputes, administration of trusts and foreclosure on mortgages, as well as adoptions, probate, family law and psychiatric cases.

Offers education and support to patients, caregivers, family, and friends affected by stroke.

Provides medical-surgical care for pneumonia, coronary bypass surgery, cardio-thoracic or vascular surgery. Includes treatments for tracheostomies, I.V. therapy, pain management, chemotherapy, and wound care management. Also provides sub-acute rehabilitation for men and women recovering from strokes, fractures, amputations, or other injuries. Services include physical therapy, occupational therapy, speech therapy, adaptive devices, prosthetic and orthotic devices, and independent living skills instruction.

Provides mental health and case management services to youth ages 12 to 20 years in the juvenile justice system who present with a full range of mental health diagnoses and await release from a juvenile justice facility. Services begin prior to a youth's release from the facility and continue in the home after release.
