Provides help with the prevention of placing a child out of the family home or ensuring the successful return of a child to the family home from an out-of-home placement. Program staff help families alleviate the immediate crisis and improve family functioning so that the risk of placement or re-placement is reduced.

PERSONS IN NEED OF SUPERVISION (PINS) in cooperation with the Dept. of Probation, provides intensive case management services to youth placed into the Persons in Need of Supervision (PINS) Diversion Program. Offers casework services to assist families in identifying issues that place children at risk and developing goals to address those concerns. Provides both home-based and school-based counseling services, crisis intervention, advocacy, and referral to other support services in the community.

Preventive services are supportive and rehabilitative services provided to children and their families to avert disruption of a family which could result in an out of home placement for the child, to enable a child who has been placed in foster care to be reunited with his or her family at an earlier time than would otherwise be possible, or to reduce the likelihood that a child discharged from foster care will return to such care.

Astor works with families who are at risk of having a child with an out-of-home placement. The program helps the family through supportive and advocacy services, create a structured family plan, and link to other needed services.

The Intensive in Home Family Preservation Program is designed to provide immediate crisis intervention and stabilization to families with child (ren) at imminent risk of placement into foster care. Provides assistance to the low income community and to the community at large in organizing issues which affect the community such as housing, education, employment opportunities, drugs and youth violence. Offers long-term psychological support for at-risk children and their parents who have mental health and parenting issues. Provides dyadic therapy (working with the child and the parent) in cases involving child maltreatment, foster care, and issues involving the criminal justice system. Also do outreach to foster care agencies, family court and other related agencies.

Provides intensive, short-term services for families in which children are at risk of being removed from the home. Workers teach practical skills for successful family life and provide counseling and other support to keep families together.

Purpose is to prevent children from being placed in either congregate residential care or foster care. Home Safe works with families that need strengthening and other assistance. Children must be referred through Suffolk County Department of Social Services. Program provides in-home parenting skills and case management services to families who have DSS/CPS involvement. Services are offered weekly for a period of six months.

Intensive case management program for substance abusing families who are at risk for recurrent child abuse and/or foster care placement. Each intensive case manager works with 8-10 families at a time for 6-9 months and provides 24 hour accessibility, frequent home visits, comprehensive assessments, development of a service plan for all family members and linkages of service needs with appropriate service delivery.

Serves children/youth at-risk for out-of-home placement due to educational, community or home-based concerns. Also serves children/youth who are currently in out-of-home placement whose discharge could be accelerated if preventive services were in place. Primary Goal is to safely maintain children in their own homes whenever possible and to greatly enhance the prospect of that permanence.

Provides services to prevent out of home placement for children whose families are referred by Nassau County DSS.. Outreach services include assessment of family concerns, intensive counseling, clinical services including individual and family therapy, referral to other necessary agencies, advocacy, parent education/groups, social/educational activities for the whole family.
